Do the bread and wine at Mass actually change into Jesus’ Body and Blood?

Most things appear as they are, but not always.

For example, at Mass we see bread and wine brought up to the altar. Yet after the consecration, the reality of those substances is no longer simple bread and wine. They have been changed at the deepest level, even though they look and taste the same.

How does this happen?
